JAMA:孕期女性应慎用抗真菌药物,否则容易流产

丹麦的研究者们指出:怀孕期女性如果因治疗阴道念珠菌病而口服抗真菌药物fluconazole的话,将会极大地提高流产的可能性。

研究者们发现:在3315名口服fluconazole药物的,怀孕7周到22周的女性中,有147名发生了流产;相比之下,13246名对照组孕妇中只有563名出现了流产。最终结果表明口服fluconazole将会提高1.48倍流产的风险。

同样,研究者们还发现口服该药物的孕妇中发生死胎的几率也显著高于对照组孕妇,但这一差异并不显著。该研究是由来自丹麦哥本哈根Statens Serum研究所流行病学系的Ditte M lgaard-Nielsen 医生等人做出,发表在JAMA杂志上。

"在更多的证据找到之前,我们还是要谨慎对待口服fluconazole的处方。尽管死胎的差异并不显著,但这一风险还需要进一步的确认"。

怀孕期间性激素分泌量的增多会导致引导念珠菌病的发生,有调查显示:美国范围内每十个孕妇中就有一个患有阴道念珠菌感染。虽然该类感染的治疗手段一般都是局部进行抗真菌药物处理,但口服fluconazole对于反复性感染以及严重的感染都是十分有效的,而关于其副作用却鲜有报道。

为了检测怀孕期间口服fluconazole是否导致流产或死胎。研究者们调查了丹麦全国范围内的新生儿、患者数据以及药物使用登记情况。他们选择怀孕7周的女性作为研究的起点,因为更早的怀孕迹象很难被准确检测。研究者们将检测对象以是否口服fluconazole 分为两组,并详细记录了他们的怀孕年龄,口服药物时的怀孕天数,并以此为标准选择相同孕龄的女性作为对照。该调查总共包括了1405663名女性。
