简要介绍:

暴露于丙型肝炎病毒(HCV)是公认的医务人员(HCP)职业风险。在暴露后应尽快(最好48h内)对源患者和HCP进行病毒标志物的基线检测。对于源患者,推荐首选检测HCV RNA,尤其对已知或怀疑近期有增加HCV 感染风险的行为,或无法评估其感染风险的源患者。推荐次选先筛查丙型肝炎病毒抗体(抗HCV),抗体阳性时再检测HCV RNA。如果HCP的抗HCV 阳性,HCP可以与源患者的检测同步进行,或应在暴露后尽快(48h内)用同一样本进行HCV RNA 的核酸检测。根据源患者的状态确定HCP 暴露后的追踪检测方案。如果源患者HCV RNA 为阳性,或抗HCV 阳性但不能进行HCV RNA 检测,或HCV 感染状态未知,则HCP应在暴露后3~6周进行HCV RNA 检测。如果HCP暴露后3~6周HCV RNA 的检测结果为阴性,应在暴露后4~6个月进行抗HCV 的最终检测。不推荐对职业暴露于血液和其他体液的HCP 预防性使用直接作用抗病毒药物。发现HCV RNA 阳性的患者或HCP时应及时治疗。
